LVFD does not process Ambulance Billing in-house. If you would like to request a copy of billing records, please contact our billing service Intermedix at 888-980-9130.
Any medical or incident report requests for official fire department records must be submitted in writing through our application with a copy of your ID and payment. Each request will cost minimum $15.00 and can be paid with either cash, check or credit card (see I WANT TO / SUBMIT). Your request will not be processed until we have received payment and the appropriate documents.
